*Please Note - insurance coverage for the Covid Vaccine can be tricky for patients 12 to 64. If you are worried about insurance coverage we recommend getting this vaccine at a pharmacy.

- Wear a short-sleeved shirt if possible.

All participants must complete the consent form at the time of the vaccine

Walk-ins are welcome but we highly recommend that you sign up ahead of time if possible.
To ensure safety for everyone at clinic, the pharmacy will do the following:
- Use proper PPE gear for vaccination.
- We sanitize all touch points in between patients
